The cost of personalizing a ketubah is $75. We encourage you to have one of our artists complete your ketubah in matching calligraphy. Please make sure that all the information on this
form is complete and correct. We recommend that you print out this form and have your
rabbi or cantor approve it. We cannot be responsible for mistakes or omissions. Please
print out the form and use Hebrew letters if you want specific spellings -- you can fax
the printed form to us at (617) 964-0489. If you use English letters for Hebrew names, we will use our judgement for spelling. If you need any help in completing
this form, we would be more than happy to talk to you.
In Hebrew, it is traditional to place the fathers name first. We therefore place the mothers name first in English. If you desire a different format, please request fathers name first explicitly. If you do not wish to have your mothers Hebrew name appear on a traditional text, please note your desire explicitly by writing in "Leave off". In Hebrew, we will include the Hebrew abbreviation for "of blessed memory" for a deceased parent unless otherwise requested.
Please allow two weeks for completion. We can also complete any
rush order you might have, but you must call us to make sure we have received your order.
After we receive your form submission, we will call you
to confirm and to arrange for payment. Please DO NOT include credit card information on
this form. If you have any special requests ( such as shipping to an alternate address or
a deadline for delivery ) , please use the comments box.
