Ketubah Personalization

Thank You for ordering your Ketubah from Caspi Cards & Art. Please fill out the ketubah personalization form below and submit it online. We recommend that you consult with your Rabbi when filling out the form. We cannot be responsible for your mistakes or omissions, so please double check the information. We will email a proof within 24-48 hours. Changes to information supplied on the form will incur change fees ranging from $10-25 per proof (this does not apply to mistakes we make). Please allow two weeks for completion. We can also complete any rush order you might have.

If you prefer, you may download the form as a PDF document and then after filling it in completely, email the form to us. If you ordered the Commitment Vows text, please use the Same Sex Personalization Form. After submitting the form, we will email you an acknowledgement with the information you submit for confirmation.

Please fill out this personalization form completely. Please do not leave anything blank. The ketubah cannot be completed without all of the information! If you do not know something or are having trouble, feel free to give us a call at 1-800-Ketubot (1-800-538-8268). We will be happy to help you. If you would like us to consult with your Rabbi, or if you have a unique spelling or situation, please note it in the Comments box at the bottom of the form. For Hebrew names, you can type the transliteration with English letters or use an (Online Hebrew Keyboard) to copy and paste Hebrew letters.

BRIDE

Bride's Hebrew Name

If unknown, just write 'transliterate'. This means that we will write the English name in Hebrew letters. You can use Hebrew or English letters.

Bride's Mother

When we fill in the Bride's name on the ketubah, we write the Bride's full name (first, middle and last) daughter of her Mother (first and middle) and her Father (first and middle). If you would like it a different way, please call us or make a notation in the comment box at the bottom.

Bride's Mother's Hebrew Name

If unknown, just write 'transliterate'. This means that we will write the English name in Hebrew letters. If you would like the name left off, please write 'leave off'.

Bride's Father
